Current Services

"All Kids" is an Illinois program for children who need affordable health insurance, regardless of family income, immigration status, or health condtion. There is no income limit and you can qualify no matter how much money you make. The insurance pays for doctor visits, including preventive care, screenings, hospital stays, prescription drugs, vision and dental care.Parents and children can apply on the same application.

If you are interested in applying please gather the folloowing things;

1. Birth Certicates for you and your children.
2. One pay stub for the last 30 days (Proof of Income)
3. Social Security numbers for each person
4. Proof of Pregnancy ( If applicable.)

Currently the Coalition is working with The National Alliance for Hispanic Health and Alivio Medical Center to form the Chicago Latino Diabetes Coalition. The CLDC would work to train health educators and provide outreach and education activities in the community.

The Door to Door Program identifies underserved and immigrant families, assesses their health needs and links them to health, social, and human services. This program provides intensive outreach and follow-up in designated areas of Chicago. Community outreach workers from community-based agencies identify those in need within their communities and coordinate needed services either through direct service provision or linkage with other health, social and human service providers.

Media campaigns conducted by the coalition are carried out within the Hispanic community with the primary purpose of increasing awareness of health risks, promoting lifestyle changes, and making resources available to members of the community.
Chicago Hispanic Health Coalition has conducted media campaigns in the following areas:
Breast Cancer / Mental Health / Diabetes / Smoking Cessation / HIV/AIDS / Nutrition / Violence / Asthma /Senior Immunizations / Kidcare/ Medicare Approved Drug Discount Card

"Por Su Salud", is a nutrition and exercise program for Hispanic women. It is designed to address the lack of preventive health programs in the Hispanic communities of Chicago. "Por Su Salud" provides practical, educational experiences that enable the participants to learn about the benefits of good nutrition and exercise so that they can take an active role in improving and maintaining their health. In addition, exercise classes are given at local Chicago Park Districts. This program is now implemented through the Chicago Department of Public Health and is offered in facilities throughout the city. Participation for the exercise classes is through referral from a provider or on volunteer basis. For a information on classes please contact us.

Racial and Ethnic Adult Disparities Immunization Initiative (READII)
CHHC is working with the Chicago Department of Public Health, the American Lung Association, White Crane Wellness, the African-American Health Care Council and other partnering organizations, to educate Hispanic seniors on the importance of vaccinating against influenza and pneumonia. The coalition works to provide outreach and education on influenza and pneumonia in the projects target Hispanic neighborhood areas. In addition to that the coalition also assists works with the group to provide bilingual services at the CDPH/ALA flu clinics during the flu season. CHHC is also available to assist other community agencies with planning their flu clinics and ordering vaccine.

CHHC offers a 6 week course which feature weekly 2½ hour health workshops each week. The programs goal is to help you learn "how to thrive, not simply survive, with chronic illness"

You will learn from trained volunteer class leaders, who have expierence with chronic illness themselves, how to....

1. How to address problems related to your on-going health condition.
2. How to communicate about how you feel with your family and friends through brainstorming
3.How to manage medication, nutrition, and new treatments.

Chicago Hispanic Health Coalition translates materials into the Spanish language. Translations are made on program materials (i.e., Lead, osteoporosis) consent forms, and protocols. Assistance in having materials translated is possible by contacting Chicago Hispanic Health Coalition
